Request a Student Chromebook
To Obtain a Student Chromebook For At-Home Use
Please follow the steps below.

2. Review & Sign Form
3.Submit Form
You can submit the form in 2 ways. Whichever is easier for you.
A. Email the Form: Email form to shoushanundorf@burbankusd.org
or
B. Submit in Person: To Main Office of your School or Have your student give form to their teacher.
NOTE:
Once the form is received, our on-site tech will provide a Chromebook for your student, which will be sent home with them for their use.
Chromebooks can not be issued without a signed Agreement Form.
